Sick Note

If you are off sick you must...

1. Notify your employer as soon as possible that you will be unable to attend work

2. If you are sick for 3 to 7 calendar days (inclusive of weekends), fill in a self-certification form on your return to work (available from your employer). If you are self-employed, obtain a form SC1 from the DSS

3. If your illness means more than 7 calendar days (inclusive of weekends) off work, obtain a medical certificate from your GP and send to your employer immediately

4. If the illness continues past the date stated on the medical certificate obtain a further certificate from your GP and send to your employer

5. Keep your employer informed about your progress and your likely date of return to work

6. There is no legal requirement to provide your employer with a sick note for a period of absence less than one calendar week. Your GP is not allowed to issue a DSS certificate for this purpose.
