New Patient Registration

If you live within the Practice catchment area you should register with the Practice as soon as possible.

You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

Forms are available to download from the link below and medical treatment is available from the date of registration.

The medical health questionnaire helps the GP or Nurse to understand your immediate medication and treatment needs whilst waiting for your medical records to transfer. 

Please remember to bring all completed documents to the practice in good time if you require an appointment.

Named Accountable GP

The Government requires GP Practices to allocate a Named Accountable GP to all their patients from April 2015.

All of our patients have now duly been allocated a Named Accountable GP but can still continue to see any GP of choice within the Practice.

If you wish to know the name of your Accountable GP please ask when you are next in contact with the Practice.

Temporary Patient Registrations

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

Disabled Patient Facilities

Disabled patient facilities are available at the Practice; toilets, car parking spaces and lift

Please be aware that we do not have our own car park. Parking can be located adjacent to the Practice, in between here and Sainsbury's. Please do not park on Bridgnorth Hospital's car park as you may be fined.
